1-9 Enrollees: Application in & digitally signed by the 20th of any month for the policy to go in force the following month.

10+ Enrollees: Business census must be submitted by the 15th of any month for the policies to go in force the following month. Group underwrite will take 24-48 hours.

If you do not have a TIN or EIN, you can use the Broward County Bar Association’s EIN and legal business name on your application.

  3. Type your zip code into the “City/County & State or Zip” box
  4. Select search method (doctor type, doctor name or facility group) and click search then “continue as guest”
  5. Click continue and select “PPO” as your network

FAQs

What are the qualifying medical questions?
  • In the last 5 years – Has any applicant ever been diagnosed with or treated for cancer, or tumors (other than full removal basal cell skin cancer), heart attack, heart surgery, angioplasty, stent, heart failure, Stroke, TIA or other serious cardiovascular disease?
  • In the last 5 years – Any Kidney disorders, lung disorders that require oxygen, blood disorders, liver disorders, organ transplants or any other ongoing organ disorders or treatments?
  • In the past 5 years – Has any applicant ever been diagnosed with a severe mental health condition (such as schizophrenia, bipolar disorder, or any mental illness requiring hospitalization), or is any applicant currently receiving counseling or therapy for a mental condition?
  • In the past 12 months, has any applicant been hospitalized overnight (other than for routine childbirth without complications)?
  • Has any applicant ever been treated for substance or alcohol dependency in the past 5 years?
  • Currently expecting children and/or in the process of adoption, surrogacy or infertility treatment?
  • Has any applicant been diagnosed with Type 1 diabetes or Type 2 with complication?
  • Has any applicant ever been diagnosed with HIV, AIDS, Lupus, Hemophilia, or any other serious autoimmune disorders?
  • Has any applicant ever been diagnosed with any neurological disorders?
  • In any applicant currently awaiting results of diagnostic testing, or scheduled/recommended for surgery, imaging or major medical treatment?
  • I attest and agree that I understand that any medication with a gross monthly cost of $700 or more per month will require PBM managed care for alternative sourcing?
  • Do you attest that you are self-employed, a sole proprietor, EIN holder or a w2 employee being sponsored by an employer (employer contributes to your monthly health insurance premium)?
  • Do you attest that all questions above are answered accurately and agree that any misrepresentation of your application will result in an exclusion of coverage for the preexisting condition attested to?
