Sole-proprietors, EIN Holders, Self-employed & W2 Employee sponsored by employer (meaning the employer contributes to the employees health insurance premium)
The PPO & EPO plans utilize the Cigna network
1-9 Enrollees: Application must be submitted & signed by the 20th of any month for the policy to go in force the following month
10+ Enrollees (Group): Census must be submitted by the 15th of any month for the policy to go in force the following month
